Scholarship Retrieval Form

Prior to requesting scholarships won at the local, state or national levels, please review the Miss Illinois Scholarship Rules that were signed as a part of the Miss Illinois paperwork prior to state competition.  Contestants should try to submit their scholarship requests at the beginning of a semester (preferably August and January) in order for the money to be paid out in a timely manner.  Waiting until mid semester is not recommended.

To claim local scholarship awards please include the following items in one complete packet and submit to your local director:
1)  Cover letter– this should include your name, student ID number and a formal request for all local scholarships that you are requesting and their individual amounts ( ex. Local winner, $250; Talent winner, $100).
2)  Miss America Scholarship Request Form– fill out the miss-america-scholarship-request-form, all items must be completed.
3) Supporting Documents- Include all supporting documents that are indicated on the Miss America Scholarship Request Form and be sure that the attached bills are well within 30 days current on the day that the request is submitted.  You MUST include all items indicated on the form or the request will not be processed and you will have to start over.  Promissory notes are required if submitting for payment toward a student loan (please remember that the contestant must be the primary payee on the loan).
4) University Forms- It is the contestant’s responsibility to provide any additional forms that their specific college/university may require when submitting a payment to the financial aid office. The Miss Illinois Scholarship Organization is not responsible for acquiring these forms.

Do not submit for your state scholarships until all local scholarships have been paid out.  Once all local scholarships are collected, to claim state scholarship awards please include the following items and submit electronically to Terri Harfst, Miss Illinois Scholarship Coordinator, at terriharfst@gmail.com:
1)  Cover letter– this should include your name, student ID number and a formal request for all local scholarships that are being requested by their name and the amount of the individual scholarships (ex. Nancy Foote Interview Award, $1000 scholarship award).
2)  Local Director’s letter– this is a letter from your local director stating that the local scholarship has been paid out and in what amount.  This letter must be included in your packet, NOT submitted to the organization individually, via email, or sent directly by the local director.  It must be a part of the packet.
3)  Miss America Scholarship Request Form– fill out the miss-america-scholarship-request-form, all items must be completed.
4)  Miss Illinois Scholarship Request Form – fill out the Miss Illinois Scholarship Request form, all items must be completed.

To collect the Carolyn Paulus Talent Award Scholarship:
The winner will be contacted by The Community Foundation of Wabash County.  The foundation will provide instructions for how to request that scholarship, but please remember that if your college/university requires that a specific form be sent along with the payment, the contestant is responsible for providing that form to The Community Foundation of Wabash County along with your scholarship request paperwork.
